2010-11 Welcome Letter to Parents

The following forms must be completed once your child has been accepted into the program. These forms must be submitted at Parent Orientation.

Enrollment Summary Form is ready for you to download, print, and complete. Bring to Parent Orientation.

Tennessee Preschool Immunization Certificate Please have your child's pediatrician complete. Bring to Parent Orientation.

Copy of Birth Certificate This must be kept in our files. If we do not already have a copy, bring a copy to Parent Orientation.

Health History Checklist Please complete and bring to Parent Orientation.

Catholic Diocese of Memphis Health Form and Medical Release. Please complete and bring to Parent Orientation.

Photo Consent Please complete and bring to Parent Orientation.

Before and After School Care If you are interested in Before School Care, please complete this form and bring to Parent Orientation.

School Supplies Bring your child's school supplies to Parent Orientation.

Please bring your child's supplies to orientation. (Not registration)

The Tennessee Department of Health (TDOH) has implemented new immunization requirements for school systems in Tennessee for the coming school year (effective July 1, 2010 ). The new immunization requirements are listed at (add the link to the forms page here). Please review these requirements to make sure your child is ready for the start of the 2010‐11 school year.

The Tennessee Department of Education requires that we must have the REVISED 2010 Tennessee Preschool Immunization Certificate for your child. Please have your pediatrician’s office copy immunization records and current physical date onto the Tennessee form (the pediatricians have the form at their office) and then bring it to your orientation meeting.

If you are unable to print any of the above forms, we will have them available at Parent Orientation for you to fill out at that time.
